Membership Application
In addition to downloading and completing the membership application, the Society will need a copy of a D.C. dental license and a written recommendation from a member of the Society. The application, written recommendation and a copy of a D.C. dental license will be forwarded to the D.C. Dental Society Board of Directors for approval. The applicant will be invoiced for dues once the Board has granted approval.
